For those of you interested in adopting a pet rat or rats, please fill out and submit this application.

For those of you interested in a rat for breeding purposes, please fill out and submit this application but be sure to put in the comments section that you are interested in breeding. You will ALSO need to fill out and submit the Breeder's Application.

In general, this application tries to cover everything I would want to know if one of my rats went home to live with you. Don't worry about what the "right" answers are, please just answer honestly. All questions are required to be answered for adoption consideration. If a particular field does not apply to you, please enter "NA" for the answer.
